Continuing Education Transcript Request Information

Due to the Federal Privacy Act we must have the student’s written consent before we can release a transcript.

Complete the Transcript Request form (PDF), giving the name and complete address of where the transcript is be sent.

The form must be signed by the student, not printed. We also need the Social Security Number and the name under which the student attended. The approximate date of attendance is helpful in locating the transcript.

If the transcript is sent to the student it will be stamped with “Issued to Student”. Some schools consider these to be unofficial and will not accept them.  If sent directly to the school it is official.

There is a $2.00 charge for each transcript requested.  The college accepts Cash, Money Order (Made payable to Wayne Community College), VISA credit and debit cards, and MasterCard® credit and debit cards. To fax a request and pay over the phone, please first fax your request to (919) 739-7133. You can submit payment to our Cashier at (919) 739-6932. Please confirm with Admissions & Records at (919) 739-6932 or (919) 739-6936 that your fax form has been received. The college does not accept personal checks for transcript requests.

Please allow at least 24 hours from the time of request before attempting to pick up the transcript in person.
